GENERAL
DEFINITION AND CONDITIONS OF WORK:
Performs intermediate human services work planning and coordinating
various services to elderly and disabled public housing residents to
assure these residents can continue to live independently. Works under
regular supervision.
This is sedentary work requiring the exertion of up to 10 pounds of
force occasionally and a negligible amount of force frequently or
constantly to move objects; requires crouching, reaching, walking and
fingering; vocal communication is required for expressing or exchanging
ideas by means of the spoken word and for conveying detailed or
important instructions to others accurately, loudly or quickly; hearing
is required to perceive information at normal spoken word levels and to
exchange or express ideas; visual acuity is required for preparing and
analyzing written or computer data, operation of motor vehicles or
equipment, determining the accuracy and thoroughness of work and
observing general surroundings and activities; not subject to adverse
environmental conditions.
ESSENTIAL
FUNCTIONS/TYPICAL TASKS:
Coordinates programs; tailors services to clients’
needs; plans programs and activities; monitors and evaluates program
outcomes; prepares reports.

Ensures the property(s) are maintained and operating
efficiently (including grounds, building interior/exterior, etc) and
reports any findings to the Property Manager.

Coordinates programs and services to link public housing
residents to various services for assistance; assists residents in
preparing appropriate forms, etc. to apply for services, UAI’s, etc.
·
Meets individually with residents to make assessments and
appropriate referrals for services, such as health insurance issues,
issues regarding food stamps, medication, etc., mental health
evaluations, independent living skills, and social security issues.

Works with community service programs to coordinate the
provision of services and to tailor the services to the needs and
characteristics of eligible residents.

Plans programs and activities; recruits program
participants; coordinates transportation to activities.
·
Coordinates
service and training programs and schedules; maintaining records and
files; prepares reports.
·
Monitors and evaluates the delivery, impact, effectiveness
and outcomes of supportive programs.
·
Coordinates with other independent living,
self-sufficiency, education and employment programs.
·
Provides and/or coordinates transportation to educational,
cultural and recreational trips and programs.
·
Prepares reports and fliers.
·
Edits and distributes monthly newsletters.
·
Assists in organizing programs.
·
Conducts surveys to determine program effectiveness.
·
Research, write, and apply for grant applications.
·
Performs other duties as requested.
KNOWLEDGE, SKILLS AND ABILITIES:
Thorough knowledge of the surrounding community and programs that assist
housing residents; thorough knowledge of the services, programs and
assistance available to elderly and disabled adults; ability to plan
recreation activities; ability to establish and maintain effective
working relationships with residents, program participants, community
agencies, the general public and associates; ability to prepare
newsletters and reports; ability to communicate ideas effectively both
orally and in writing; ability to prepare and maintain records;
ability to maintain confidential information;
ability to use a computer
for spreadsheet and database management
effectively.
MINIMUM EXPERIENCE AND EDUCATION:
Any combination of education and experience equivalent to graduation
from an accredited community college with major coursework in social
work, administration, or a related field, and considerable experience
working with disabled clients or in the social work field. Considerable
experience working with elderly and disabled families.
SPECIAL REQUIREMENTS:
Requires
possession of a valid appropriate driver’s license issued by the
Commonwealth of Virginia and insurable under the vehicle insurance
policy of the Authority. Must be able to work a flexible schedule.
Personal transportation is required.
